Project Overview
CareSouth Medical & Dental is a private nonprofit Federally Qualified Health Center that provides comprehensive, affordable health care. Founded in 1997 as Capitol City Family Health Center and renamed CareSouth in 2016, it operates from a main location in Baton Rouge with clinics in Donaldsonville, Plaquemine, Zachary and Shreveport.
Modiphy built a website that helps patients across multiple clinics find care, book appointments and access the patient portal.
The Challenge
CareSouth offers medical, dental and behavioral health care plus many specialty services across several Louisiana communities. Patients needed to find the right service and location quickly, book online and access forms and sliding fee information in English and Spanish.
Our Solution
We built service pages for medical, dental and behavioral health, plus dermatology, HIV primary care, pediatrics, physicals, podiatry, urology, women's health, WIC, pharmacy, mobile unit and transportation services. The site includes appointment scheduling, patient portal access, patient forms and rights, bilingual sliding fee information, a vaccine page, events, careers and a patient survey.
The Result
Patients can now find the right CareSouth service and clinic, book appointments and access their records online, making affordable care easier to reach across the region.
