Project Overview
Southeast Community Health Systems is a private, not-for-profit Federally Qualified Health Center with 21 service sites across East Baton Rouge, Livingston, Tangipahoa and St. Helena parishes. It provides medical, dental, dermatology, behavioral health, gynecology, podiatry, WIC, pharmacy, nutrition, telehealth and vision testing, and is Joint Commission accredited and NCQA certified.
Modiphy built a website that helps patients across a large network find the right service and location.
The Challenge
With 21 sites and many services, patients needed a simple way to find care near them. SCHS needed a site that organised services and locations clearly and served patients of all ages.
Our Solution
We built service pages, location listings, school-based health center information, pharmacy details and telehealth information, with accreditation and contact details.
The Result
Patients across four parishes can now find the right service and nearest site and access telehealth.
